America has the bones of a really great healthcare system.
I'm on Medicaid. The thing about Medicaid is that it's restrictive. Doctors can refuse to treat me because they legally can't. In Tennessee, it's illegal to charge money to a Medicaid patient if you're in some kind of system. I had to stop seeing my psychiatrist even though she didn't take Medicaid and was allowed to take money from me. No prescription she wrote could be covered by Medicaid even though they would be if someone else wrote them.
Medicaid is great if you're low income. But you're not getting the best care possible. My primary care physician is at a walk in clinic.
How states like Tennessee handle Medicaid is disgusting. Everyone who is poor should be on Medicaid. Full stop. But unless you're a mother, a child, or legally disabled, you have to get cervical cancer or something else extremely specific to get it here.
There are people who have insurance through their work or insurance through the marketplace who have it just made. Before my mom's work switched companies, her insurance made me feel like a high roller. We had to pay quite a bit, but the care felt great. The only denials I got were for my depo shot (they paid every other time because you're not actually supposed to use it to treat premenstrual dysphoria) and my hospitalization for monitoring for my psychogenic non-epileptic seizures. The bill was like five figures and we did the paperwork for the hospital to forgive it.
If insurance companies weren't outright evil, the American system would work beautifully with just a few tweaks. (Insurance being tied to a job is less evil if having insurance through your work just means you get to see the coolest and highest tech doctors for free and losing your insurance and going through the Affordable Care Act marketplace means you still get great care.)
If the insurance companies were good and there were some tweaks, then the American system would be great.
Medicaid would be open to everyone with low income (bump up that ceiling so more people get it!) The ACA marketplace would be open to everyone not on Medicaid and scale the price based on your income though subsidies and stuff like that. Employers can sweeten the pot by offering super plush insurance plans, but you'd more than survive on the ACA (which would be scaled to income so that people aren't going into debt over paying for their insurance).
So in conclusion... Stop it Tennessee. Bad. Give poor people Medicaid. I wouldn't be so mad about being treated like I can't decide I want to spend my money on my own care if you weren't gatekeeping this mess. It was traumatic watching my mom work herself to the bone at a sub-minimum wage job just to qualify for the ACA marketplace for us to have insurance.
